Moving forward, all appointments should be scheduled either here on my website or by signing into your Charm PHR Account, which you will need to establish for access to your health records and telemedicine appointments as well as for easiest and most secure communication with me about your health needs. If you have an account with my clinic already, you can easily schedule below or through your CHARM Portal. If you do not already have a portal account, you can try scheduling an appointment below or click on the patient portal link to create a portal and it may allow you to do so directly (there have been some tech difficulties for some trying to do it this way, while the system has let others due so. If you encounter any issues making a portal account, please text your full legal name, DOB, sex assignment on your insurance card, and your email address to 971-258-0991 and you will receive an email invitation to create a PHR account.
